Education Health Care Plan - Aged 16-19

Working at Entry Level or GCSE Level

Attendance in Y11 is at least 90% or above

Complex additional needs including Autistic Spectrum Condition (ASC), Specific Learning Difficulty (SpLD) Speech, language and communication needs (SLCN), Social, Emotional and Mental Health Needs (SEMH) and medical needs

How to Apply: Consultation through Medway SEN.

Parents and Learners should add Rivermead Post 16 as their choice during their Annual Review.


Course Summary

EPR+ is a tailored post-16 provision designed to support learners with Special Educational Needs and Disabilities (SEND), offering a broad and practical curriculum that promotes independence, confidence, and employability.

The programme includes core academic subjects such as Maths and English at GCSE or Functional Skills levels, alongside life skills like cooking and personal development through PSHE and RSE.

Learners also engage in Occupational Studies, exploring areas like arts and crafts, carpentry, fitness, and video projects. Physical Education and meaningful work experience—ranging from animal care and gardening to community projects—further enrich the curriculum, ensuring students gain real-world skills in supportive, inclusive environments.
